📋 Tanggung Jawab Pekerjaan

• Coordinate post-transplant care plans, including medication schedules, follow-up appointments, and specialist referrals
• Monitor patients' progress and identify any potential issues or complications
• Provide education and support to patients and their families on post-transplant care, including diet, exercise, and emotional well-being
• Collaborate with healthcare providers to ensure seamless communication and continuity of care
• Manage patient records and maintain accurate documentation in electronic health records (EHR) systems
• Assist in the organization and coordination of transplant-related workshops, seminars, and support groups
• Stay updated on the latest advancements in kidney transplant care and best practices in patient management

🎓 Kualifikasi & Syarat

• Bachelor's degree in Nursing, Healthcare Administration, or a related field
• Minimum of 3 years of experience in a post-transplant care setting, preferably in kidney transplant care
• Certification in post-transplant care or a related field (e.g., Certified Post-Transplant Coordinator)
• Strong knowledge of kidney transplant care, including post-transplant complications and management
• Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families
• Proficiency in English, both written and spoken
• Familiarity with electronic health records (EHR) systems, such as Epic or Cerner
